5/25/2023 EnchantaVaughn Approved 1.Behavior: Upon arrival, Ben-Nita was observed at home helping mom around the house. Ben-Nita was in a pleasant mood and communicated well. Ben-Nita and mom communicated thoughts and concerns well with each other. Ben-Nita interacted safely in the home, while QMHP-C was present. Ben-Nita made constant eye contact with authority figures while communicating her thoughts and feelings concerning today’s topic of discussion. 2. Behavior: Ben-Nita presented off task behaviors at times within the session as Ben-Nita was getting upset with QMHP-C when discussing her behaviors on learning how to communicate better with others. 1.Intervention: QMHP-C went to the home for the purpose of meeting with each family member for family session and meeting with Ben-Nita individually to do social skill activities. QMHP-C engaged in conversation with each family member about their day and how they were feeling. QMHP-C reiterated the expectations and rules for family session. QMHP-C listened to mother verbalize any issues or concerns in the home. QMHP-C reiterated with each family member about completing assigned chores in the home. QMHP-C implemented an activity to increase self-esteem by receiving positive comments from mom and Ben-Nita. The activity was also completed to hear positive comments about themselves 2. Intervention: QMHP-C provided Ben-Nita with encouragement to demonstrate the positive behaviors and expectations they had been talking about. QMHP-C encouraged Ben-Nita to communicate to her mom when she needs help. QMHP-C praised Ben-Nita for staying on task. QMHP-C complimented Ben-Nita for utilizing acquired skills, paying attention, and focusing on the importance of the assignment. QMHP-C supported Ben-Nita displaying positive social interaction with peers and authority. 1. Response: Ben-Nita displays normal behavior in home setting. Ben-Nita exhibited a positive attitude toward mother and sibling. Ben-Nita said it’s boring and she is having challenges with certain activities and needs help. Ben-Nita said she has been doing good but got in trouble for talking back. Ben-Nita knows what is expected of her and is trying to make better choices. Ben-Nita verbalizes positive feelings about herself but recognizes areas she needs to work on. Ben-Nita smiled and hugged QMHP-C. Ben-Nita was observed in her seat and raising her hand. Ben-Nita interacted well with others and demonstrates good team building skills. Ben-Nita displays good attitude by saying “thank you”. Ben-Nita did an excellent job by showing interest in the session by asking appropriate questions. Response: Ben-Nita laughed when the counselor attempted to observe her behavior. Ben-Nita stated that when she is not interested she does care what others think or feel. Ben-Nita shared that she tries to remain in a more positive state of mind but people that she does not like speak to her and make her more upset. Ben-Nita was receptive to role play once she calmed herself. Ben-Nita expressed that she was not being rude to counselor on purpose she just needed to bring down her aggression. Ben-Nita was attentive during demonstrations of positive behavior as evidenced by giving eye contact and asking questions while examples were being provided.Client Zar stated reported that he doesn’t care about the water and the floor in the bathrrom. Zar reported that Ms. West should have clean the floor up her damn self. Zar went on to report that he was going hurt someone if they kept talking about him. Zar reported that everybody continues to talk about his behaviors around the house. ZAr reported that he was tired of the “bullshit” and was going to “slap” someone if they kept saying his name out their mouth.Ms. Shelton reported that she was happy about receiving services for Anaya . Ms. Shelton reported that Anaya really need it to help her with reducing some of her behaviors within the home. Ms. Shelton reported that Anaya is out of control and really needs help. Ms. Shelton went on to report that she is overwhelmed by Anaya behaviors within the home,school, and community. Ms. Shelton reported that she is out of ideas when its comes to dealing with Ayana.Ayana Reported that she was tired today. Anaya reported that her mother and father are a “trip”. Anaya stated that she will try to reduce her negative feelings towards her mother and “fall back” a little. Anaya reported that she was going to look into staying with her grandmother for a few weeks. Anaya went on to report that she will reduce the amount of times of utilizing profane language towards authority figures within the home, school, and community.Anaya reported to counselor that Ms. Shelton and others were being a little overboard expressing their concerns about her behaviors. Ayana went on to report that she was a little upset when had been informed that services were discontinued. Anaya informed counselor she had a few “run ins” with family member and others within the last few days prior too todays services. was receptive to the empathy displayed by QMHP but continued to exhibit anxious behavior as long as she was expected to discuss his triggers to strong emotions
